This survey is for people that want to potentially come an administrator for the server. After you have filled this survey out, Forsaken will notify you if you eligible for administrator. After that, you will have to pay for administrator. (See Admin Page for more information)

Necessary after you've bought something

This survey is required if you have bought anything. In this survey, it will ask you for certain information (e.g. name color). If you have failed to complete this survey, your items WILL NOT arrive.
